INTRODUCTION: The scope of the Brazilian Health System, an important advance was the organization, planning and health care in a regionalized manner. To understand in depth the arrangements and dynamics of Health Care Networks in the different realities of the country, it is necessary to know the organization of the health system and the health policies implemented for each region. AIM: To present case studies that used 5W2H tool to perform strategic actions for the implementation of five lines of care for chronic non-communicable conditions recommended by the Ministry of Health, in two health regions of northeastern Brazil. MATERIALS AND METHODS: This is a two-case study with a qualitative, descriptive and exploratory approach. The 5W2H tool was used to identify priorities and guide decision-making regarding the definition of intervention plans and main implementation actions. RESULTS: In both regions, there was the participation of 86 municipal and state managers and supporters and 89 health service professionals and civil society representatives.
The prioritization of problems and actions were listed following the themes:(1) primary health care; (2) specialized care; (3) hospital care; (4) emergency care; (5) regulation; (6) management. CONCLUSIONS: The evidence presented demonstrates an effective tool for prioritizing action planning, constant and relevant participation of managers throughout the process and significant interest of health professionals in improving clinical management.
